They only use the RAT if there are no other power sources, if all engine generators and the APU are non-functioning. They'll turn on the APU inflight as needed, including in non-emergency situations. Depends on the aircraft though, not all aircraft can use the APU inflight.
Yep. I imagine this was more like a compressor stall. Lots of smoke after a Big thump or bang.
