Looking to help someone get silver status as they've flown ~35k miles but have not been invited.
Just pay the fee, is my recommendation. It's not a lot and MPC has changed to be more closely aligned with revenue-based anyway. If your friend is worried about saving $100 MPC is probably not the right program! It's pretty bad value all around, with the upshot (IMO) that they treat their own DM members extremely well, and do have stunning J lounges for SL (limited) and GOs.
I say this with the knowledge I have never - not once - seen these freebie forms handed out nor even know what they look like, as an 8 year Diamond member. I am willing to believe these forms exist because I see people memention them every now and then here in FT. But I don't think it's like some "travel hack" where anyone can just get it and you're dumb if you pay the fee. Because I presume I would've seen them handed out in hundreds upon hundreds of flights in J and F. So I don't think it's as easy as, "hey I've flown before let me in for free!", and everyone who pays the fee is just some lackey.
If your friend has flown a lot, you're better off asking for a status match to SL than trying to get Green for free. That would be much better IMO and perhaps even more likely.
