They way mine works is after booking the flights there's a bank-branded page on the travel insurance website where I enter my CC number and purchase data and it verifies then asks for personal details and issues the actual policy on their letterhead, etc. All with EuropAssist's hotline on it.

Includes medevac as well, and includes certain sports, even bungy jumping, riding a motorcycle (if licensed) and scuba diving (if certified) which is quite good.
