Were any of your missing segments continuation/direct 1 stop with the same flight # on two segments? If so it will only get posted as destination A-C, not A-B, B-C

If not, try resubmitting your request with airline code AC and I bet it'll go through (since your ticket undoubtedly shows all AC flight codes).
