This policy seems to be buried on the KLM website. Has anyone had any experience actually getting this refund?
I was able to get a refund from the KLM central office in The Netherlands, I sent them an email with the details and later on someone called me, but actually what they suggested was to call them directly the next time. Also these days facebook is a good way to get in touch with airlines. If your journey starts in the US I would try with the US office first.
