Been on any trips with serious delays or cancellations in the past few months? (I'm assuming not, since you would've mentioned this). Otherwise, it could be a case of mistaken identity -- your account was up on some agent's screen when s/he meant to credit somebody else (as occasionally happens with SWU and mileage deductions).
