Regarding the 'no promo due to another promo within 2 years':

The wife got a letter yesterday saying that 'since she had had another promo within 2 years' that she would not be getting any avios. I was about to cancel her card when I went ahead and did a little checking. First, it had been more than 2 years. Second, they had already deposited the first 50k points in her (and our household) account.

We will see if the next 25k shows up.
