I had this happen once. I phoned it in. Phone agent put me on hold for a long while, then said they were VERY sorry for the misunderstanding and issued a new cert. No explanation of why it happened. (In my case, both I and my wife volunteered and IIRC her $500 cert came out okay, but mine was mysteriously cancelled.)
