It's certainly happened. When I used mine in Austria, shops that could DCC would always ask if I wanted to be charged in US$ even though euros are one of the 10 currencies supported (and I can pay that euro balance in US$ for less than 0.2% spread).

The one time I actually got taken was VAT refund after a summer trip/exchange program to Europe. I presented my ICBC card because it natively supported euros and pounds. Global Blue refunded me in US$ minus
their spread. Don't know who to blame for that one, though.
