After 5 or 6 weeks of badgering, and about 21 phone calls (zero of which GoToGate promised to return, despite promising every time that they'd call me back within 24 hours), GoToGate finally managed to rebook my friend into a business class seat, albeit not on the most convenient connection - she now has a six hour overnight at DOH for her onward connection to BKK. Most frustratingly, if they'd seen to this problem when I'd first contacted them about it, they'd have been able to book her on that immediate connection.
Everything seems to have settled down now, though weirdly they moved her into the D fare bucket before adjusting the PNR again to I class. And then the recent equipment change took place, so now she's on a 777 rather than the A380 she would have had.
All of this has left a very bad taste in my mouth about booking with GoToGate in the future, but the moral of the story, as mlqsko very helpfully pointed out, is that you really have to ride the OTA - hard - to make them want the problem (you) to go away. I'm pretty sure everyone in the GTG call centre knows my name now.

Glad you got this sorted out, even though not a perfect solution but remember, GoToGate were pretty nice to all of us with that
special fare
PS: 388 to 77W switch is all QR, the OTA is not to blame.
