I called LH and they said it's not mandatory. But just to check that if you guys have flied with LH to somewhere which requires passport and visa. Is it possible to submit the information at check in counter i.e. showing my passport?

Also noticed that there's a blank in the form called issuing country in "visa information". My visa was issued by Embassy of Ireland in Beijing. So if I have to fill in the form, do I put China or Ireland?
