I was wondering, if the cat needs to stay in its cage, how do you deal with the toilet issues?
My own cat has been known to soil the cage on its trips to the vet.
No, it wasn't too stressful -- I think that the droning plane noise/vibration, being under the seat, and reduced air pressure calms them down and encourages them to sleep. I also used a pheromone spray that is supposed to calm down cats, but I don't know how necessary it is.
I didn't feed him for the 18 hour trip, and offered him some water halfway through, but he didn't drink any.
For toilet issues, I had a few puppy pads with me. I just had one on the bottom of the carrier just-in-case (though luckily he didn't need them this time). Puppy pads are like large, flat, paper diapers and absorb liquid well.
