The room will be booked under my name. I know i can access lounge and +1, but can my friend with gold access the lounge as well and bring in the 4th person?

Generally, only the registered Gold member and +1 get free access. There can be a charge for any more. Now, IME, I have never been charged for extra guests staying at the same property, in my room or otherwise. YMMV and what's going on at the property and how you act while there may decide if they wish to go beyond the rules and grant access to everyone you know. Just don't get upset if they follow the rules.
