In all cases the outlets are worn from use such that the plug is no longer firmly held in place. My solution is to position my device so that the cord is "pulling" my plug upwards and towards my seat. I find this occurring most frequently when traveling on the older 737s in the fleet.
I've had similar issues with plugs not holding well in airplane outlets (sometimes appearing to stay in but not actually charging); a good workaround I got from FT is to use a cheap european 2-pin adapter -- the prongs are longer and fatter and hold better in the multi-country capable outlets.
