I find the burn rates very generous. But I fly 80%-90% JAL so the 130% Diamond bonus boosts my earnings quite a bit. Without that, maybe not quite as good. BA and AA also qualify for an elite bonus on miles for usage. The rest of OW does not.

Regarding the JGC application form, by default they send it via snail mail. Upon qualification you can also call JMB and ask them to email you the form. The latter is what I did and if you email the completed form back to them it's faster. My JGC application was processed and updated in my JMB account within 2 days.
