Does this apply only to preclearance, or to the entire airport, or how does it get enforced?

Switzerland doesn't require you to remove shoes. They're EEA but not EU.

I also don't remember having to take shoes off in NZ. Note that NZ has 3 levels of security: none for turboprops, light security for domestic-jets (i.e. liquids allowed), full security for international.
