It seems that the carriers deal with end users, e.g. passengers through their FFP's. Much easier to just zero out and close the account with loss of status and all that entails than worrying about proving up fraud.
Never heard of anybody having anything bad happen for a single hidden city fraud. OTOH, I don't know that people brag on the Internet about bad stuff.
