My interpretation of the baggage info page is that the extra bag with the AmEx card applies only to silvers on domestic flights. They would get one bag but can check two with the AmEx card. On flights to Japan in the main cabin all medallions get the same, which is one bag (50#) over the standard allowance, giving a total of two.
