Instead of an inflatable pillow, I recommend a Bucky for long-hauls. It prevents neck strain while sitting (use the airline-provided pillow for lumbar support), and flattens out nicely for sleeping. sierratradingpost.com was selling them at half-price, but they currently appear to be out of stock.

I've found Silencio SDI-101 earplugs to be the most effective, comfortable, and easy to use (and not lose).

(Although not related to the First Class experience, I always take saline nasal spray for long-hauls to avoid a parched nasal cavity.)

Finally, as recommended by a prominent FTer, accessorize your stylish outfit (wearing jeans or, Aquascutum forbid, sweats, is a decidedly coach experience and not what you desire) with an ascot -- guaranteed to attract the FA's attention and enhance your service!
