1. Flight headed to ORD,
2. Subject to ground stop,
3. Pax required to offload after boarding commenced,
4. Wheelchair pax had to wait for wheelchair.
5. Wheelchair pax traveling with two companions.

That pretty much identifies the flight and thus the Captain. If it doesn't, it's down to 1-2 and all UA needs to do is ask around a bit and identify the guy.

Other crew who do read this board will remember this one for a long time.
