Yes, its from sCO - there were no sUA planes with DTV that I can remember. Given the push for streaming device entertainment recently, I'd bet no more DTV installs happen. I seem to remember reading on here that the DTV system isn't owned by UA. LiveTV owns all the gear, or some arrangement like that.
In the past I've been annoyed by the pay for map problem too - thought about posting about it but never did. It seems like screens that aren't activated/paid for play a channel selected by the FAs. Perhaps they should select the moving map channel?
Is there some type of contract that requires United to keep using it? Seems even the 737s with the new seats have it . . . why not just convert those planes to wifi ondemand entertainment?
