Yes, this is the incident--I'm sure of it. My boss was from South Bend, so ORD-SBN makes much more sense than ORD-IND. It sounds like the details were significantly exaggerated in the re-telling around our office, but this is definitely it.

I'm impressed--you guys are great detectives! I knew someone here would have the answer.
