Have flown a few times with Apartments full and half full, no difference in the very good service, (getting almost personal now as some crew remember me), cannot answer for showers, grannies do not shower mid stratosphere!

However (and this is deviating from the OP's post), there has been a recent trip report from a ft member, commenting on lack of service in F Apartments. I have never experienced this myself, when the doors are closed, giving me my much valued privacy, if I need something I buzz the FA, with no guilt. I am reasonable with my 'buzzing', and would be horrified if the FA's kept opening my door to see if I needed something! Just my pennies worth.
