I went annual for the amount I travel, and also for those grey areas when I stay on for a few days after a business trip. Cost is £75 per year, which is quite expensive (in UK terms), but it does have the highest medical limits I've found for N America - I'm paranoid!

Most UK insurers will only insure UK residents, but it may be worth checking.
