In my experience so far usually you need a key to access the lounge. Sometimes, they ask for the name of the primary on the room. The process allows for some flexibility on who is with you because it is relaxed.
I'd hate to think of a future scenario where people start abusing it and the relaxed atmosphere goes away. . .
