I earned one a couple of months ago - but still waiting for it. You can check status on the Barclay site. Mine showed to be processed Feb 18th, and that was 3 months ago.
It is supposed to be good for one year (from issue date I believe).
Good for US domestic travel but with some black out dates. Works at any fare for first ticket, is $99 + about $50 in taxes (varies by ticket). You can get 2 $99 tickets on the same PNR as base ticket.
