It's something we would like to automate, but it's complex due to all the rules and such. Generally Costco is the best route to go with since as you point out, the additional driver can be anyone (not just a spouse and/or another Costco member). That helps with Avis/Budget and Enterprise/National/Alamo but not with Hertz/Dollar/Thrifty or any of the smaller guys.
AAA/USAA/AARP/etc. can help as well but of course like Costco is depends on if you are a member. We're working on allowing you to save these preferences, so this should be easier over time. There are also of course a couple of companies in Hawaii that offer a free additional driver as part of their negotiated rate. These are special deals they have negotiated with the various car rental companies directly. They can sometimes make sense, especially if the additional driver is not a spouse, but it depends on the situation.

Great, thank you so much. I had no idea (until both
cblaisd and you posted it) that I could email you with these kinds of things. Often I am alone and sometimes your rates are just so good that I will forgo having Mr. Fink drive if it is for just a week or so, but if we have a 3 week or more rental, it just isn't feasible. Good to know that you can help. And I am a member of Costco, AAA and AARP so it would be a great feature to be able to specify this on your site.
And as an aside, I have turned so many people on to you and every single one has gotten a lower rate from you. They are, of course, thrilled.
