The beautiful Thing at MIA (and YVZ) is that
even first-time ESTAs can use the kiosks there. And yes, People of
all Ages can use them (though Kids below 14 are more likely to get an X due to biometric verification issues).

For GE it is different as it is done electronically and I understand shown on the receipt you get out of the machine.
Regardless of how you entered (other than by land), the I-94 is electronically recorded and the admission class is shown on APC Kiosk receipts. So they may very well not stamp your passport after using APC (though at LAX TBIT, on 13 January, they seemed to be careful about stamping even US citizens). Comes down to the airport or even the specific officer you get.

Well on the new ESTA you would declare any Name changes (due to marriage for example) so I cannot imagine this should be an issue if she's done it correctly. However, the fact that she's a first-time ESTA (=first entry on current passport) means she'll get an X, if let into the Kiosk Queue at all (at MIA and YVZ, perhaps also MCO, this isn't an issue as even first-time ESTAs can use the kiosks there)

Depends on port of entry. At MIA and YVZ (also MCO according to rumours) first-time ESTAs
can use APC (first-time ESTA=first entry on current passport, or ever)
