The surcharges you seem to have in mind are imposed by airlines (carriers), not by cities. If you're paying in $, the final prices won't vary much from one airline to another, unless a particular airline is offering a sale, and what matters to you is the total price of your ticket, not how the components are classified. Award tickets using AA miles fall into a quite different category: award flights on BA have prohibitive surcharges (imposed by BA, not LHR), those on IB have lesser surcharges, and those on AA/AB have no such charges.
