I'll add that the REALLY limited item is the Kaiseki menu from KIX and HND; if you're an op-up on that flight, it is unlikely you'll receive it. Since most of the passengers order it IME. I've flown those routes both as revenue and op-up and only got the Kaiseki menu when I was a revenue pax; each time I was op-up it ran out before getting to me.
