I've only had one as a OWE. It was on RTM-LHR from WT to Club Europe. Got to the gate, scanned my boarding pass, got the negative beep, and she handed me a fresh one for 1F (which had "upgrade" scribbled on it as well.)

The flight was full due to the cancelation of the previous night's flight.
