Why is it so random? Are the FA just doing them if they feel like it? The hot towel is not as big of a deal as the inconsistency of practices between flights and flight attendants.

Yes, does seem pretty inconsistent—sometimes not that hot, sometimes not happening at all—but I forget that it exists at all until it happens, and then I forget about it immediately ensuing. You can try complaining in the surveys.
