Often there's a promo so that if you buy a RT ticket, you get two days of free subway rides, IIRC not including the "private" lines.
AFAIK you cannot make a reservation for the ride FROM NRT, but you should make a reservation for the return trip TO NRT just in case the bus you need is full. I usually do this at my hotel upon arrival.
