My personal favorite hotel in Antigua is Casa Florencia. It is owned by a Guatemalan woman who has lived in the United States and she speaks fluent english. The hotel has only 10 rooms and is one of the cleanest hotels you will ever stay in. Just look at the reviews on tripadvisor. It is the top rated bargain hotel on TA at $46 single/$54 double.
You can book it on booking.com, but if you email them, you will get a prompt response and the booking will go smoothly.

I always stay in room #6, which is on the top floor with views of 3 volcanos. If you decide to stay here, just ask for rooms 6 thru 10 as they all have volcano views. They are priced a little higher than ground floor rooms.
It is not considered a Luxury hotel, but it is a really nice property.
