I recently canceled an award ticket with paid taxes/fees using AmEx online, phone agent was able to cancel the ticket but couldn't get the refund nor the miles back.

During transit in IST, ask the ticket office to resolve it, took them 20 minutes (between the agent and the manager) to resolve this (sounds like they have really antiquated reservation system) as they tried to refund the fees toward different non-AmEx card.
