Expect Thai immigration to be inconsistent. There was no such requirement for me inbound 2 days ago at DMK. In fact, as you note, the form itself is free form where address is concerned. I simply scribble hotel name, without address. Many times it isn't even the hotel I'm staying at

I might stay at a hotel on the first night and then move on... doesn't matter I guess. I've had several agents be sticklers for location (accepting BTS stations with no relation to the hotel itself) but never have had any demand a phone number yet even though they supposedly are demanding it.
