I just received a mailing from Virgin Atlantic Flying Club with a little booklet thanking me for choosing to "spend another year with the Virgin Atlantic World Elite Mastercard credit card" and a card telling me they've posted 15,000 miles into my account for spending $25K. Of course that deposit happened back in the beginning of October and I canceled the card at the end of November, but better late than never.

Also, hokiegl, it answers one of your questions -- the miles posted in October on my anniversary and the annual fee posted some time after that, so I received the miles before I needed to pay the fee. I wasn't able to get a statement credit -- see my post above.
