I've proceeded to what looks like the final booking page on all 3.

Travelocity wanted an additional $35 CAD + GST for delivery of paper tickets.
Destina wants an additional $21 CAD for delivery of paper tickets.
BMI does not seem to charge either for paper tickets or for a credit card fee (not to say something won't pop up). But BMI does not allow a place to add one's Aeroplan number: only Diamond club, Lufthansa, or SAS. ??

Edit: I found BMI's extra 3 pound fee in the terms.
