Center bathrooms on widebody a/c have enough room for doing the deed. Specifically 763 mid section center bathroom on some airlines has more than enough room. Other airlines divide this bathroom in half with a removable partition when the bathroom is required by disabled/ wheelchair passengers.
that reminds me of the original config on UA's 767-200s ~1982: the two mid-cabin lavs (one on either side of the cross-aisle around Row 12) had an outward-opening door that could actually be latched in the open position against the wall of the opposite lav, so a wheelchair passenger had plenty of room to maneuver
