At least Thrifty now tells you up front. In old days, the fees were hidden until you got the car. €200 sounds very cheap to me coming out of an airport in Italy. Italian Airport rental fee is 14% by the way.
I have never rented a car in Europe with a tag from a different country. They must have to take them back to where they came from.
I have always used Autoeurope, and when I just plugged in 4 days, MXP-MUnich, i get a low rate of €241 complete.
