There are a few that are known to treat serious medical problem from the 1900's.
Hot Springs Arkansas for one.
The biggest thermal area is the least known. Thermopolis, Montana. How is that for a name?

I feel a REAL spa has to have mineral spring waters that you drink for the INTERNAL effects.
