Yes you have to fill in two forms to link your FFP if you want to redeem for KF and AM respectively. In previous years this used to be one combined form, but since DBS added Air Asia (who in the right mind will want to redeem for a LCC anyway... but that's another topic of discussion
) they separated into individual forms.
Thereafter actual redemptions can be done online or by phone.
The fee of S$42.80 per year applies from the moment of first FFP linkage, regardless of whether you link only one or more FFPs.
