I think you need to apply for the Five Star status through AMEX - just having the card or adding its details to Hertz account will not change the Hertz Account status. Apply through AMEX and then AMEX arranges the account upgrade with Hertz

When your account is upgraded to Five Star, it does show on the Hertz account. When you click on "My Account" it will show something like

Gold Plus Rewards Five Star
