I think the spa/fitness access issue is cultural. It should never be assumed that any hotel in Japan will provide this. Most Japanese guests would never ask for access after check out time, unless they intended to pay to have access to the room until departure.
I don't see why there would have been any "service recovery" re the bag being forgotten in the taxi. As long as they retrieved the bag and brought it to your room reasonably promptly, what recovery would you expect?
I think they got lucky that the taxi driver stopped and she managed to get the bag. It seems the issue here is that they forgot the baggage or missed it somehow. They shouldn't miss it in the first place.
