The book & upgrade system is flaky. Best to book CW then upgrade the flight/s with Avios. Can be done all at the same time over the phone.
Unless you have the dubious pleasure of getting the Indian call centre who consistently insist that 1 department can do cash bookings and 1 does avios and both do their best to ping you over to the other one.
