Sites know who their "neighbours" are and which ones to hand off to (including roamers and home sites.) It would certainly add complication to set up each individual site to know where the femtocells are, and further complicate it telling the femtocells which are their neighbour sites are.
since all these devices require a GPS signal and location to function, it would seem that they should automagically know exactly where they are. That location information is relayed back to the mothership and I am sure incorporated into at least the company's view of "network" coverage.
