OP's concern is legitimate.
I don't think AMEX will cover a dishwasher as it can be considered as a permanent addition.
My suggestion is to get the extended warranty from the retailer. I know it sucks. But this is the only way AFAIK. If AMEX closes the door on this, I can pretty sure that many will do the same or similar.

In order to become a fixture, the item in question must be installed in the real property.
For example, your actual house unit and fences are fixtures (Unknown to most people, the house unit is considered as a personal property).
In OP's case, OP will be stuck in the wording "permanent addition" as generally speaking, no one will take the dishwasher away after selling the house.
