So I went ahead applied for my 2nd A+ and was approved instantly. Guess I'll have to wait and see whether I get the bonus.
On a side note, my wife (who is a current A+ card holder) tried applying for A- today and was denied for some reason.
Update/Data point: Got my 1st card in Nov, 2014, earned the bonus, used it, then cancelled in June, 2015. Applied for 2nd card last month, was instantly approved, recently hit the spending req, and my bonus posted yesterday. FYI, my 1st card still shows up on my account management page as a closed account.
