I came here to answer my own question. The refund department did not return my call. They actually looked up my e-mail (probably from the MileagePlus # I left in the voicemail, I did not leave my e-mail address in the voice message). The reply asked me to call their reservation department. I was instructed to call the refund department by the reservation department in the first place!
I have turned to the online web form. No reply so far. I am afraid I need to escalate the complaint level.
