This could be rectified by the train manager resetting the system but if the train is full and he's the other end (or on the other train in a double header) you can be without a toilet for ages. Same goes for the not very excellent trolley service which if (you're lucky enough to have one is) stuck at one end due to a full train you had to walk to and then discover that they were out of what you wanted because "there is only a limited amount that we can fit into the trolley". Oh and don't mention hot food if the trolley can't make it down the train as it doesn't happen, they have to collect it from the 1st carriage and can't leave the trolley unattended. That and the smell is something else, I did one trip in 1st where the entire carriage just reeked - it was really bad and you obviously couldn't open a window to get the smell to go away. I was very surprised when the train manager came in, apologised for the nasty niff and said this wasn't a 1st experience. He said he was declassifying 1st so that we could get a refund of the extra cost of 1st over a regular ticket.
Okay to clarify, the door locks itself and the out of service notice appears
after you have left the toilet not during.
